The complexity of the tumor microenvironment limits the understanding of cellular communication mechanisms, particularly the role of exosomes in cancer progression. The objective was to analyze exosome-mediated cellular communication and its influence on tumor dynamics. A quantitative, non-experimental, correlational approach was applied, based on a systematic review of scientific sources and international organizations from 2021 to 2023, using Pearson correlation and multiple linear regression. The results showed a strong positive correlation (r = 0.82) between exosome concentration and tumor progression, as well as significant explanatory power of exosomal microRNAs (R² = 0.68) in cancer aggressiveness. An increase of up to 35% in cellular invasion and a 28% reduction in apoptosis were also identified, confirming their role in metastasis and therapeutic resistance. Consequently, exosomes are established as key mediators in tumor microenvironment reprogramming and as a basis for developing diagnostic and therapeutic strategies.
